Our Commitment to Exceptional Care

The Same Therapist. The Same Standard. Every Visit.

The best therapy happens when a clinician truly knows their client. We match each Support at Home participant with a consistent therapist who visits regularly, tracks progress and adapts the program as needs change. No rotating staff, no generic plans, no shortcuts just focused, professional allied health delivered in the place you call home.

We focus on:

Why Support at Home Participants Choose Back2You Therapy

We work specifically with older Australians who need more than short-term treatment. Our approach is built around functional progress, clear communication and therapy that fits into everyday life — not just clinical outcomes on paper.

Therapy That Fits Your Life

Every program is built around your real routine, home environment and personal priorities. Improvements are designed to translate into daily tasks, movement and independence — not just numbers on a report.

Clear Goals, Regular Reviews

Therapy is guided by clear goals and reviewed regularly as your needs change. Progress is shared with families and care managers so everyone stays informed and aligned throughout.

Coordinated With Your Care Team

We work alongside families, carers, care managers, GPs and other health professionals to ensure therapy is connected to your broader Support at Home care plan and delivering real outcomes.

Frequently Asked Questions

The Support at Home program provides funding through classification levels based on your assessed care needs. Each level provides a different budget that can be used across a range of approved services including physiotherapy and occupational therapy. Your funding level is determined through a My Aged Care assessment. If you believe your needs have changed since your last assessment, you can request a review through My Aged Care at any time.

Yes. Under the Support at Home program you have the right to choose your own allied health provider. You are not required to use a provider recommended by your care manager or package provider. Back2You Therapy works with Support at Home participants across Brisbane and the Gold Coast who have chosen us as their preferred physiotherapy and occupational therapy provider. Contact us directly and we will work with your care manager to get started.

The biggest difference is consistency. Many allied health providers rotate staff regularly, meaning you see a different clinician each visit. At Back2You Therapy, we match each Support at Home participant with a dedicated therapist who visits them consistently over time. That continuity allows the therapist to build a genuine understanding of the client, track meaningful progress and deliver care that improves with every session rather than starting from scratch each time.

Yes. Many of our Support at Home clients access both physiotherapy and occupational therapy as part of their care plan. Our physio and OT work closely together, sharing information and aligning their programs so that every aspect of your care is working toward the same goals. This is particularly effective for older adults managing neurological conditions, recovering from a hospital stay or working toward greater independence across multiple areas of daily life.

Your first session is an initial assessment carried out in your home. Your therapist will take the time to understand your current health status, your home environment, your daily routine and the goals that matter most to you. They will also liaise with your care manager to ensure therapy is aligned with your Support at Home plan. From that first session, a personalised program is built specifically around your needs and circumstances. Sessions run approximately 45 to 60 minutes.

Yes. If you are currently receiving physiotherapy or occupational therapy under the Support at Home program from another provider and would like to switch to Back2You Therapy, the process is straightforward. Contact your care manager to discuss updating your care plan and then reach out to our team on 1300 858 555. We will handle the transition as smoothly as possible and ensure there is no unnecessary gap in your therapy.
